Hankel
An \(m \times n\) matrix \(A\) is called a Hankel matrix if there
exists a vector \(b\) such that
\[A(i,j) = b(i+j).\]
The following routines create an \(m \times n\) Hankel matrix from the
generate vector, \(b\).
